Secure Login Forms

• Declaration of Major Form: Use this form to change, add or drop majors and minors. 
• Declaration of Candidacy for Degree: Use this form to apply for graduation. Please submit this form no sooner than one year prior to anticipated graduation date. 

Nursing Student Forms

• Nursing Application Audit Request: Print this form and submit it to your designated campus representative (see instructions on the form for details). Please submit once you have completed all of your pre-requisites and you have applied to the Associate in Nursing program.

Business Certificate Forms

• Intent to Pursue: Print this form and submit it to the evaluations department to declare your intent to pursue one of the four business certificates Columbia College offers.  
• Application for Awarding of Certificate: Print this form and submit it to the evaluations department to apply for award of business certificate. Please submit this form once you have enrolled in the final course work to complete the certificate.  

Appeals and Transfer Request Forms

• Course Equivalency/Substitution/Waiver Appeal: Meet with your academic advisor to request a direct transfer credit equivalency, permission to substitute required course work with a course you have already completed, or to request a degree requirement waiver. This form must be submitted by your academic advisor via formal, internal review channels, and only after an official evaluation of your academic record has been completed. 
• Graduate Transfer Credit Request Form: Print this form to request transfer equivalency credit for Master-level course work. Submit the form to your academic advisor (AHE campus students) or to the evaluations department (Columbia, MO, campus students).
• MBA Accounting track (MBA.ACCT) Undergraduate Course Substitution Form: Print this form to request use of an undergraduate course in place of one of the MBA.ACCT course requirements. Submit the form to your academic advisor (AHE campus students) or to the evaluations department (Columbia, MO, campus students).

Graduate Rank and Classical Program Forms

• Request for Classical General Education Notation: Students who completed their bachelor's degrees on the Classical General Education Track may request notation of this accomplishment on their official transcript. Please print this form and submit it to the evaluations department for processing.
• Request for Letter of Graduate Rank: Graduates of Columbia College may print and submit this form to request a formal letter of class rank. Please complete the form and submit it to the evaluations department. Be sure to include recipient address.
