Appeals and Transfer Request Forms
Course Equivalency/Substitution/Waiver Appeal
: Meet with your academic advisor to request a direct transfer credit equivalency, permission to substitute required course work with a course you have already completed, or to request a degree requirement waiver. This form must be submitted by your academic advisor via formal, internal review channels, and only after an official evaluation of your academic record has been completed.
