All new students in the day program are required to submit a completed Columbia College Medical History form and show proof of two vaccinations for measles, mumps and rubella (MMR). Failure to submit these documents to Health Services will prevent students from registering for subsequent semesters. For more information, contact Health Services at (573) 875-7432. Completed forms may be faxed to Health Services at (573) 876-4456.
